In order to assist the internal and external client of NedFleet, herewith a brief explanation of the forms available for completion on the system:
This form is divided into 3 sections of which the application form is a required section.
The application form records the contact information.
The vehicle information form records the vehicle information. The completion of this section is optional.
The billing point form records billing information, and which section/s within an organisation will be responsible for the paying of the accounts.
This form is used to apply for new NedFleet cards.
The full vehicle details need to be recorded on this form
The contact persons’ full details and telephone number is required.
The contact person will be notified once the card is ready for collection or delivery.
The complete form should be submitted to NedFleet before 12 noon for the Fuel Card to be available for collection within 48 hours.
This form is used to record any changes to the following sections.
Replacement Card
Vehicle registration change
Vehicle driver name change
Cost Centre or Branch change
Card Cancellation Notification
To be completed when giving authorisation to delete a vehicle from the system.
To be completed when notifying NedFleet of a lost or stolen Fuel Card.
The client must notify NedFleet as soon as the loss is known to prevent fraudulent usage.
